We’re glad to have you here and hope this onboarding resource will be valuable during your university career! We are excited to get you started as the latest member of the Trailblazer team! We will need a few tasks completed and your information before your employment begins.

Required Employment Documents

FOR HIRING SUPERVISORS HIRING A NEW EMPLOYEE

Use the Part-Time Assignment Form (PDF), and submit the form to the Utah Tech HR office as soon as possible. Incomplete forms could delay the new hire process and defer payment for services.


FOR NEW EMPLOYEES

All onboarding employment documents must be completed.

  • Check Your Email – You will receive an email from the Droplet System.
    • Click the link in the email to access your onboarding checklist.
    • Complete the required tasks. These may include uploading documents, completing forms, or reviewing training materials.
  • Didn’t Get the Email?
    • If you do not receive an email within 24 hours of your hiring confirmation, please contact your hiring supervisor immediately.

Please note the employment forms are time-sensitive and must be completed promptly to ensure you are eligible to begin work and receive timely payment. To avoid delays in your start date or paycheck, please complete the required tasks and submit the requested information before your employment begins. Submit the items to the HR office and upon the successful completion of the new employee hiring process, you will have an email account created if you are new to Utah Tech University.

FOR HIRING SUPERVISORS RE-HIRING AN EMPLOYEE

Use the Electronic Personnel Action Form (EPAF) System completed through the EPAF system, which allows for paperless employment transactions such as:

  • Re-Hiring an Employee
  • Job or Pay Adjustments
  • End of Employment

FOR RETURNING EMPLOYEES –

Online Employment Application

Utah Tech University no longer accepts paper employment applications. New employees submit an online application and submit a brief resume. Follow the instructions when completing. Answer all questions completely and correctly.

Online Employment Eligibility Verification (I-9)

Complete the online Employment Eligibility Verification (I-9) on or before your first day of employment. The I-9 form is used to verify the identity and employment authorization of individuals hired for employment in the United States. BOTH employees and employers must complete the form.

  • Complete Section 1 of the online Employment Eligibility Verification (I-9) on or before your first day of employment. Once Section 1 is complete, employees bring the original acceptable forms of ID to the UT Human Resources office to complete Section 2.
    • Accepted Documents: New employees must bring their original unexpired acceptable documents into the Human Resources office. Expired documents, copies, faxes, digital copies, or unacceptable documents cannot be accepted and will delay the payment process. For details about accepted documents, see the U.S. Citizenship and Immigration Services.

Bring completed forms with supporting documents to the Human Resources office in the Burns North Offices (enter from the north side, downstairs to the left). A member of the HR staff is available to complete the I-9.

UT Direct Deposit at myUT

Use the Utah Tech Direct Deposit Form to participate in the direct deposit program. Deposits may be made to a bank or credit union into a checking or savings account. All employees require Direct Deposit; see University Policy 342 for additional information.

  • Log into your myUT account and log in with your University credentials.

Direct deposit notifications are emailed to your university email account by default. However, you can change this on myUT in the Personal Information tab if you prefer to use a different email address.

W-4 Employee’s Withholding Certificate

The W-4 tax allowances you claim determine the federal and state taxes that are withheld from your gross pay each pay period. Fill out numbers 1-7. Sign on the Employee’s signature line and date.

To determine the correct number of allowances you should claim on form W-4, complete the Personal Withholding Allowances Calculator provided by IRS.

Statement of Acknowledgement

Employees are to review and submit the completed documents to the Human Resources office.

You may use the visitor parking spaces on the north side of the Anthony Burns Arena marked with green pavement striping. Visitor-marked stalls are for visitors to UT only, and not for students, faculty, or staff at any time for any length of time. Any student, faculty, and staff vehicles found parked in a Visitor stall will receive a citation for Parking in a Reserved Zone or Parking Stall.

New employees must bring their original unexpired acceptable documents into the Human Resources office. Any expired documents, copies, faxes, digital copies, or unacceptable documents cannot be accepted and will delay the process of payments. SPECIAL NOTE:
Any missing items will delay processing and confirmation, and Utah Tech will terminate an employee who fails to produce an acceptable document or documents, or an acceptable receipt for a document within three business days of the date employment begins. Thus, we appreciate your assistance when completing the employment forms promptly. Upon the successful completion of the new employee hiring process, you will have an email account created if you are new to the University. IMPORTANT DISCLOSURES & ONLINE TRAINING

  • Part-Time Employment Policy 310: Definition of types of non-benefited part-time employment and outline employment guidelines
  • Right to Know: Available to you according to the Higher Education Act and other consumer information requirements.
  • Health Insurance (PDF): In compliance with the Affordable Care Act, employers must provide information about the availability of Health Insurance Marketplace coverage. Please retain this document for your records.
  • Minors on University Premises Training (YouTube): Those part-time employees paid by a Pink Contract Services Form (and not hourly on a part-time employment form), who interact 1:1 with minors, will need to complete the Centralized Scheduling Minors on University Premises Training video (approximately 10 minutes). This training fulfills the requirement in place of the SafeColleges (Vector Solutions) training listed below.
  • Vector Solutions Learning Management System, also known as SafeColleges Online Training (link): All new and returning non-student employees must complete assigned SafeColleges online training. These trainings help ensure the safety and well-being of our students, staff, and faculty. Training assignments typically appear after Human Resources enters your job information into the Banner system. Using your Digital ID and password, you can access the SafeColleges system through the UT Human Resources homepage.
  • FERPA Training: Part-time employees are now required to complete their FERPA training through their Vector Solutions account. To do so, they should navigate to the course catalog, search for ‘FERPA,’ and follow the course instructions. Upon completion, employees must download the certificate and email it to their supervisor.
  • Confidentiality Agreement Form: Certain part-time employees working with a significant amount of confidential data, or non-personal confidential data related to general operations and finances, will be required to complete a Confidentiality Agreement Form as a condition of employment. This form will be part of hiring packets available through Droplet and submitted by the employee to Human Resources.

Digital ID Number

Your university Digital number is generated once your position is created in the Human Resources office. If you have not received your ID number by your start date, please contact: hr@utahtech.edu  There couuld be missing documenation from your employee file.

If you are visiting campus before your hire date, park in the green-marked Visitor stalls. New hires can only purchase a parking permit on or after their start date.
