Accident/Incident Reporting

EMPLOYEE WORK-RELATED INJURIES
    Utah Tech University provides workers’ compensation coverage for faculty, staff, and on-campus student employees who sustain a work-related injury or illness. For additional information, see UT Policy 345.

    Workers’ compensation may provide benefits for eligible work-related injuries or illnesses, including medical care and wage-replacement benefits when applicable.

NON-EMPLOYEE INCIDENTS & INJURIES
    Students, visitors, vendors, and contractors should report accidents, injuries, unsafe conditions, or other incidents that occur on campus as soon as possible.
    When completing the Incident/Injury Report, provide as much relevant information as possible, including details about what occurred and, when appropriate, photos, drawings, additional narratives, or other supporting information.
Students Working at Clinical Facilities:

A special request is required for students working at approved clinical facilities to ensure applicable state and facility liability coverage requirements are met. Contact Human Resources before the student begins work for assistance.

REPORT A HAZARD OR UNSAFE CONDITION

Unsafe conditions and damaged or broken equipment should also be reported promptly to the appropriate University safety or risk management office.
