Utah Tech University offers dental insurance through EMI Health Dental.

There are two dental plans to choose from:  Preferred Plan and the Traditional Plan.  Both plans have two available networks included:  The Advantage Plus network (the largest network) and the Premier network.  Both of these networks are included with each plan.  To search for dental providers, create an account or login at emihealth.com.

EMI Dental Change Form

To add, remove, or make any changes mid fiscal year, login to NEOED / eForms and submit an EMI Dental Change form.  Attach any necessary documents on the form for a mid-year change (e.g. birth certificate, marriage license, etc.).
