Employment forms are time-sensitive, so to avoid any delay, select one of the following items:

For a NEW Part-Time Hire:

Use the NEW Part-Time Assignment Form at UT (PDF), and submit the form to the UT HR office to Amy Hofer as soon as possible. Incomplete forms could delay the new hire process and defer payment for services.

For all other part-time changes:

Use the Electronic Personnel Action Forms (EPAF) System completed through the EPAF system, which allows for paperless employment transactions such as:

  • Re-Hiring an Employee
  • Job or Pay Adjustments
  • End of Employment

UT no longer accepts paper employment applications. New employees submit an online application and submit a brief resume. Follow the instructions when completing. Answer all questions completely and correctly.

Employment Eligibility Verification (I-9)

The I-9 form is used for verifying the identity and employment authorization of individuals hired for employment in the United States.

Accepted Documents.
New employees must bring their original unexpired acceptable documents into the Human Resources office. Any expired documents, copies, faxes, digital copies, or unacceptable documents cannot be accepted and will delay the process of payments.

UT Direct Deposit

Use the UT Direct Deposit Form to participate in the direct deposit program.

Deposits may be made to any bank or credit union checking or savings account. See UT Policy 342 for additional information.

  • Health Insurance: As part of the Affordable Care Act, employers are required to provide a notice of the availability of a Health Insurance Marketplace Coverage.
  • Minors on Campus Training: Those part-time employees paid by a Pink Contract Services Form (and not hourly on a part-time employment form), who interact 1:1 with minors, will need to complete the Centralized Scheduling Minors on Campus Training video (approximately 10 minutes). This training is in lieu of SafeColleges Trainings listed below.
  • SafeColleges (Vector Solutions) Training: New and Returning Non-Student Employees are required to complete assigned SafeColleges trainings. The safety of our students, staff, and faculty is of the utmost importance. That’s why we’re pleased to offer the SafeColleges online training system for our employees. If these trainings are not completed 60 days after the hire date, a letter of non-compliance will be placed in the employee’s personnel file. A link to SafeColleges is available from UT Human Resources homepage; enter your digital-id and password to login and see the required trainings. The trainings are typically loaded after your job information has been entered into Banner by Human Resources. Questions or concerns regarding the Employee SafeColleges trainings should be directed to Rae Matalolo (435-652-7594) or Travis Rosenberg (435-652-7521).

Any missing items will delay processing and confirmation, and UT will terminate an employee who fails to produce an acceptable document or documents, or an acceptable receipt for a document within three business days of the date employment begins. Thus, we appreciate your assistance when completing the employment forms promptly. Upon the successful completion of the new employee hiring process, you will have an email account created if you are new to UT. Regularly check your UT email account for important campus information.
